National System for Incident Reporting (NSIR)

The National System for Incident Reporting (NSIR) collects patient safety incident data from participating Canadian health care facilities. Authorized individuals within these facilities can then access the national database to inform local and system improvements in health care delivery.

BCPSLS has been submitting de-identified medication incident data to NSIR since 2015 and radiation treatment incident data to NSIR-RT since 2020. BC is the only province to submit data via electronic data transfer.

NSIR and NSIR-RT were developed by the Canadian Institute for Health Information (CIHI), an independent, not-for-profit corporation that produces health information to help decision makers improve the health care system.

NSIR bulletins contain information on medication and radiation treatment incident reporting and analysis for sharing and learning across Canada.
